Once downloaded, please fill out the registration form in its entirety, collect all donations from each team member, and send it to us via regular mail to the following address:
NDP Lacrosse/1000 Chopper Circle/Denver, CO/80204
Proceeds from team registration will go to a fund that will assist Colorado pre-collegiate lacrosse players to realize their dreams.
Suggested per player donation is $25. Any player that donates $40 or more will receive a tournament T-shirt.
Checks should be made payable to "Colorado Lacrosse Foundation". In the memo line of the check, please write "Colin L. Cassidy Memorial Fund".
Each team will play a minimum of 3 games during the day.
Registration deadline is September 1, 2010!!
